Information for Faculty and Staff – IRS Form 1095-C Update

Updated Due Dates per IRS: 2019 IRS Form 1095C Employer-Provided Health Insurance Offer and Coverage

For benefits eligible faculty and staff employees who have consented to receive their IRS Form 1095-C Employer-Provided Health Insurance Offer and Coverage Statement in electronic format, the 2019 forms will be available by March 2, 2020, per the IRS’s updated due date.  A Hornet Hive announcement will be sent as soon as the 1095-C is available.

To access your Form 1095-C or provide your consent, click on the “Employee Profile” link in Web Advisor and then the link for “Tax Information”.  

All tax information is housed within this link with separate tabs for Forms W-2, 1098-T Tuition Statement, and 1095-C Employer-Provided Health Insurance Offer and Coverage documents, as applicable.

For those who have not provided consent for the electronic format by January 26th, 2020, hardcopy 1095-C’s will be mailed to your preferred address by March 2, 2020.

You can also update your address online, click on the “Employee Profile” link in Web Advisor and then the link for “My Profile Update”.  

Physical possession of Form 1095-C may not be required for tax submission. Please consult a tax professional for individual advice.
