Employee Return to Work Questionnaire

Kalamazoo College will follow a three-phase plan to gradually return to working on campus beginning in July 2020. The plan’s goals are to protect the health of employees, help ensure the well-being of the community, and carefully and deliberately prepare for the fall term. Kalamazoo College will phase in a return of staff over time in a coordinated process to ensure appropriate social distancing, availability of PPE (personal protective equipment) and careful assessment of the success of our efforts. Expanded staffing will be tightly controlled and coordinated to mitigate potential risks and ensure the safety of our community. The information below will allow the College to make data-driven return to work plans to provide departments with basic office coverage while recognizing the challenges individuals face as we phase back to on-campus work including childcare and high-risk medical conditions. 

Accommodations Procedure

If upon completion of this form, you indicate you have other concerns about reporting for on-campus work (e.g. childcare, living with someone who is high risk for COVID-19, etc.), your supervisor, in consultation with Human Resources, will follow-up to discuss options. 
Upon completion of this form, if you voluntarily disclose as being personally at high risk for severe illness from COVID-19 (per CDC:  https://www.cdc.gov/coronavirus/2019-ncov/need-extra-precautions/people-at-higher-risk.html) Human Resources will contact you to begin the interactive accommodations process set forth by the Americans with Disabilities Act (ADA).
