COVID-19 Leave Self-Reporting

The Families First Coronavirus Response Act (FFCRA) went into effect April 1, 2020.  This act provides employees with paid sick leave and/or expanded family and medical leave for specified reason related to COVID-19.  Information about employee’s right under the FFCRA can be found at

Under the FFCRA an employer may be eligible for certain payroll tax credits related to COVID-19. 

In an effort to better track which tax credits the College may be able to use The College is asking all employees to self-report the information below.

COVID-19 Self Reporting

  • Your Employee ID Number from your Kalamazoo College ID Card. It is 4-6 digits in length
  • BW = Biweekly MO = Monthly
  • Have you been advised to self-quarantine because of an infection or close contact with someone who is infected? If yes, when did the self-quarantine begin?
  • Are you experiencing symptoms of COVID-19 but have not yet received a medical diagnosis? Typical symptoms include fever, coughing, and shortness of breath. You are strongly urged to remain at home and be in communication with your medical provider if you are experiencing symptoms of COVID-19. If yes, when did symptoms begin?
  • Are you caring for a close relative for illness or for a child(ren) due to lack of daycare for COVID-19 related reasons? If yes, please indicate type of care, for whom, and the date the care began.
