Students must be vaccinated in order to be on campus beginning in the summer 2021. The following resources ensure that students, faculty, and staff partner to ensure that our campus is as healthy as possible.

Students who were vaccinated on-campus or who already reported their vaccination status to the Student Health Center, do not need to complete the form below as your vaccination status has already been recorded.

Students who need to report their vaccination status, please have the following handy.

• Your student ID number
• The reason why you expect to be on-campus during summer 2021
• The date on which you expect to begin your summer responsibilities at K
• A digital (jpg, pdf, etc) copy of your vaccination record

Note about paid positions: If this is your first paid job on-campus, please visit the student hiring checklist page. Please consult with your supervisor to determine if your summer responsibilities are paid or unpaid.
