Pilot: Supervisor Feedback

Thank you for participating in this pilot opportunity to increase team communication and supervisor effectiveness. All submissions are anonymous, unless the submission includes identifiable information. The goals of this initiative are

  • To provide formative information to supervisors on what is working well and what could work better, and
  • To give individuals a venue to provide honest feedback.

  • Consider sharing S.M.A.R.T. information: Specific, Measurable, Achievable, Relevant and/or Timely
    • Specific: Directly related to the unit’s mission or purpose
    • Measurable: Feedback could include how a goal could be measured
    • Achievable: The topic should be within your supervisor’s control
    • Relevant: The topic should support established goals
    • Timely: The topic should be relevant to the period of time
