By submitting this form, a copy of the evaluation will been sent to the supervisor email address included on the form. Here are the steps to follow after submitting.
- Use the evaluation to facilitate a meaningful conversation with your employee.
- Attach additional comments.
- Obtain confirmation of receipt of the evaluation from the employee by either forwarding it to the person or requesting their signature.
- Send a final copy to the Office of Human Resources.
