Performance Review

By submitting this form, a copy of the evaluation will been sent to the supervisor email address included on the form. Here are the steps to follow after submitting.

  1. Use the evaluation form to facilitate a meaningful conversation with your employee.
  2. Attach additional comments.
  3. Obtain confirmation of receipt of the evaluation from the employee by either forwarding it to the person or requesting their signature.
  4. Send a final copy to the Office of Human Resources.
