- If enrolling in coverage, you’ll need dependents’ date(s) of birth, address, and social security number.
- Changes are effective July 1, 2023.
- Everyone must actively participate in enrollment, including recent new hires.
- Instructions are posted under Files on Microsoft Teams under the Open Enrollment Team.
- Assistance is available virtually on Microsoft Teams under the Open Enrollment Team. Message us your questions or email HR.Benefits@Kzoo.edu.

Unum Life, Accidental Death & Dismemberment and Accident Insurance Enrollment Information
Unum packets will be mailed to your address on file with the College. Please return your enrollment packet if you wish to make changes to your Voluntary Life or Accidental Death & Dismemberment Insurance. You may also enroll in Accident Insurance, if you wish to, through Unum. Accident Insurance must be renewed annually if you enrolled previously.
Unum enrollments may be scanned and emailed to HR.Benefits@Kzoo.edu, mailed via interoffice at the College or;
